Referral to Hospital

When you consult with your GP, he/she may think it is appropriate to refer you to a specialist at one of the local hospitals or clinics. At the Wycliffe Medical Practice we operate the 'Choose and Book' system which is a Government initiative allowing you to choose your hospital or clinic and in some cases allow you to book an appointment with a specialist. 

If any issues arise during your visit or stay at Hospital, the Patient Advice and Liaison Service (PALS) can help with any questions, concerns or difficulties you may have about any of the hospitals services then contact PALS.

Choose and Book

Choose and Book is a new service that allows you to choose your hospital or clinic and in some cases book a date and time with a specialist.

When your GP decides that you need a referral for a specialist opinion they will discuss which speciality you need to be referred to and then give you an information sheet to take to the ‘Choose and Book’ Advisor who will discuss with you the choice of hospitals or clinics for that speciality and if available the appointment date and time.

The benefits of Choose and Book

 You can choose from at least four hospitals
 You can choose the date and time of your appointment.
 You experience greater convenience and certainty, so the appointment you pick is convenient for you.

Choosing an appointment place and time

The Choose and Book Advisor will discuss which clinics or hospitals are available for your referral.

If you are unable to book an appointment before you leave the surgery then the Advisor will give you an appointment request letter.  This shows your appointment reference number, the number for the ’Choose and Book Appointment Line’ and a list of hospitals and clinics available for you to choose from.  Every appointment has its own reference number.

You can book an appointment either by phone, internet or by calling your chosen hospital.

Once you have booked your appointment, you will receive a letter detailing the place, date and time of your appointment.  It will also include any special instructions you need to know.

If you want to change your appointment, then you can do so on the internet or by telephone.  You will need your appointment reference number.

Patient Advice Liaison Service (PALS)

‘PALS’ have teams in all the local hospitals.  It is a free and confidential service that helps Patients, their relatives or carers.  They can answer questions about your experience with the hospital and can offer advice and support.

All hospital staff are happy to help.  However, you may prefer to speak to a Patient Advisor as someone who is not directly involved in your care.  PALS aims to provide a voice for patients and works with hospital staff to help resolve your concerns.
