How to Register
We are always pleased to welcome new patients.
You will be asked to complete and sign a new patient form prior to registration with the Practice. It would be helpful if you could provide your current NHS Medical Card.
Patients register with The ’Wycliffe Medical Practice’ rather than an individual GP. You may still ask to be seen or treated by a particular GP. We will make every effort to ensure you do see the doctor of your choice, but this may not always be possible, particularly if you need to be seen urgently.

If you live in our practice area and wish to register as a patient at our surgery then we need certain information from you and each member of your family so as to be able to trace your previous medical records. We advise you here on how you may register as there as several methods of doing so.

You can fill in the online request form and we will send you the necessary doumentation (not yet in operation).

You may call into our surgery, bringing with you your NHS medical cards from your previous NHS doctor.

You may call into the surgery without bringing with you your NHS medical cards from your previous NHS doctor. This may arise if you cannot find your cards or if you do not have any e.g. if you have recently arrived in the country. This is less preferable since it may take longer to trace your medical records and you will have to fill in more paperwork, so please find your medical cards if you can.

You may ring the surgery during opening hours and request a registration pack which will be posted to you.
Note that we cannot guarantee that we shall accept you as a patient since, for example, it may be that your address is not in our catchment area.
