How to Register
We are always pleased to welcome new patients.
You will be asked to complete and sign a new patient form prior to registration with the Practice. It would be helpful if you could provide your current NHS Medical Card.
If you would like to register you can either download the questionniare below or alternatively request a copy from one of our receptionists
Patients register with The ’Wycliffe Medical Practice’ rather than an individual GP. You may still ask to be seen or treated by a particular GP. We will make every effort to ensure you do see the doctor of your choice, but this may not always be possible, particularly if you need to be seen urgently.

If you live in our practice area and wish to register as a patient at our surgery then we need certain information from you and each member of your family so as to be able to trace your previous medical records. We advise you here on how you may register as there as several methods of doing so.

You can download and fill in the online questionnaire below

To register, you may call into our surgery with your completed questionnaire and NHS medical cards from your previous NHS doctor. Alternatively you can post them and we will contact you once we have received them.

If you do not have your NHS medical cards from your previous NHS doctor then please come into the surgery with your completed questionnaire. This may arise if you cannot find your cards or if you do not have any e.g. if you have recently arrived in the country. This is less preferable since it may take longer to trace your medical records and you will have to fill in more paperwork, so please find your medical cards if you can.

You may ring the surgery during opening hours and request a registration pack which will be posted to you.
