We would like to ensure patients are involved in decisions about the range and quality of services that we offer.  Our Patient Reference Group has advised us of areas we should focus on and we have devised a questionnaire to obtain the views of as many patients as possible on these important issues.

We suggest that you save the questionnaire onto your computer, complete it and email back to us.  Alternatively, if you prefer you may print it and return the paper copy to us.   We also have printed paper copies available in the waiting room.

We hope to get your responses by 22 February 2012. The Patient Reference Group will meet in early March to discuss the results of the survey and to work on an action plan based on these results.  If you would like to attend this meeting please let us know – either by speaking to a member of our reception team or by emailing us at Wycliffe.medicalpractice@nhs.net
